Appendix 1

1.      Contractor shall be responsible for preparation of the Operation Designs and Post-Well Reports and get the Designs and Reports approved by COMPANY prior to commencing any specific operation during contract execution phase. The Post-Well Reports shall be submitted to Company within one (1) months after the operation is completed.

The operational designs including but not limited to the following

1)        Workover Operation Design

2)        TCP Perforating Design

3)        Work Fluids Design

4)        Acidizing and Flowback Operation Design

5)        Wireline Operation Design

6)        Slickline Operation Design

7)        Other Designs if required by COMPANY

The Post-Well Reports include but not limited to the following

1)        Post-Well Report for Workover Operation

2)        Post-Well Report for Acidizing and Flowback Operation

3)        Other Reports if required by COMPANY

2.      Contractor shall be responsible for provision of preliminary surveillance reports (or evaluation reports) within 1 week after the operation is completed, and shall be responsible for provision of Final Surveillance Report (or evaluation reports) within 1 month after the operation is completed.

3.      Contractor shall deliver six (6) hard copies and six (6) electric copies (CD-ROMs) to Company for document achieving and Auditing purpose.

4.      The Operation Designs and Post-Well Reports shall be provided with proper bookbinding and covers meeting reasonable publication standards (please refer to the attached for reference).
